St. Christopher Mother's Day Out Enrollment Form 2024/2025

To register your child, please follow these steps:

1) Review the St. Christopher Mother's Day Out Program Information and policies.

2) Print out the St. Christopher Mother's Day Out Medical Form and have it signed by your doctor. This form is not due at time of registration but must be turned in by 1st day of MDO.

3) Complete the form and pay the registration fee.

4) If you have more than one child to register, you will need to fill out a separate form for each child.

Please read through the following info and complete the form to enroll your child in the program. If your child's class/age group says "WAITLIST", please email an enrollment request to our MDO Director at Tami@stchrischurch.org.


There is a registration fee of $100.00 which is required to complete enrollment. **If you are adding your child to our waitlist please select the waitlist option under the registration fee.








* St. Christopher MDO Classes are offered from
   September through May. School hours are 10:00AM
   until 2:00PM Tuesdays and Thursdays, and we will
   mostly observe the CCISD school calendar dates. 

* Enrollment into the Explorer 3's and Adventurer 4's 
   classes require students to be fully potty trained. 


* Please note that our tution for the 2024-2025 school
   year is $200 per month, and the rate for siblings is
   $180 per month. 


* There will be more paperwork that is required. You
   will receive a mail out in August to the physical
   address you provide below with required paperwork
   to bring to Meet the Teacher in September.
